Latino Health & Wellness

Our Center of Excellence for Latino Health (CELH) was established to serve as a hub of health and wellness information for Latino families. As a trusted medical and community resource across New Jersey, CELH strives to increase community partnerships, promote health literacy, increase access to care, and eliminate health disparities throughout the life cycle. We focus on addressing the unmet needs of the local Latino community for specific disparities of medical conditions, such as diabetes, hypertension, hyperlipidemia, and obesity and cancer.

Our Latino Clergy Council is dedicated to assisting pastors, their families and congregations by connecting them to health care providers and services in collaboration with the Greater Newark Health Care Coalition Grant funded Health Faith Based Initiative.

Our goal is to improve health care delivery to the local Latino community by:

  • Providing and facilitating primary, emergency and specialty, inpatient and outpatient medical care.
  • Focusing on transitions of care, chronic disease management and end-of-life care.
  • Looking at clinical and other market research to understand patient needs and improve health outcomes.

We plan to:

  • Ensure that Latino patients seen our facilities are aligned with a Primary Care Physician (including Pediatricians and OB/Gyns), to ensure consistent medical care.
  • Create a directory of health care sources that will include primary and specialty providers, outreach workers, health educators, social workers and other community organizations that provide needed services.
  • Assist with enrollment into quality insurance products through the NJ State Health Insurance Exchange program, as well as NJ Family Care (Medicaid).
  • Develop a Latino youth mentorship program that will touch on a variety of health care issues and promote health care career opportunities.
