Transitional Care in New Jersey

Maintaining Your Health After Leaving The Hospital

Just because a patient is discharged from the hospital doesn't mean they don't need assistance monitoring and managing their health. At our facilities, we are dedicated to providing continuous, accessible transitional care for our patients so they can complete their recovery and maintain their well-being at home.

What is Transitional Care?

Transitional care is the continued monitoring and treatment of patients after they leave the hospital and transition back to their everyday lives. Often, these patients require long-term care to address their medical condition, but even those recovering from short hospital visits should have the knowledge and resources they need to make their recovery as complete as possible.

Our transitional care includes:

  • Dietary counseling
  • Laboratory testing
  • Pharmacy services
  • Diagnostic Radiology
  • Wound care
  • Recreation programs
  • Social work
  • Pet therapy
  • Ostomy education
  • Diabetes education
  • Congestive heart failure treatment

Our goal with transitional care is to ensure that our patients have the tools and awareness they need to maintain their health. Our physicians, nurses, and counselors can help these patients identify medical warnings signs, manage a prescription regimen and schedule follow-up appointments over the course of their recovery. All patients will receive detailed contact information as well in the event they need to reach our medical professionals from home. In the end, we wants to make sure our patients are cared for as they work to make a complete recovery.
