The RWJBarnabas Health Patient Transfer Center (PTC) will coordinate your patients' transfers to the advanced tertiary and quaternary care hospitals of RWJBarnabas Health.

24 Hours a Day, 7 Days a Week, 365 Days a Year

The PTC is staffed by a qualified nurse coordinators, specialists, and dispatchers who will triage and initiate the patient transfer process for your patient into an RWJBarnabas Health facility.

Access to RWJBarnabas Health Specialists

The PTC’s nurse coordinators will be able to seamlessly connect you directly with the most appropriate physician to discuss the appropriateness for transfer.

The RWJBarnabas Health PTC was designed by physicians for physicians and other clinical specialists involved in the transfer process.

Types of Patient Transfers

  • Acute Care Transfers
    One call to move adult, pediatric and neonate patients in, out, or between hospitals in the health system, schedule transport and complete physician to physician hand off.
  • Direct Admits
    Direct admissions from physician offices, SEDs, urgent and immediate care facilities into the appropriate hospital location.
  • Physician Consults and Telemedicine
    Connecting two physicians for a consult with or without transfer involvement.

Streamlined Transfer Process

  • Sending physician places call to 732-926-4111 to initiate transfer.
  • Call is answered and triaged by an experienced critical care transfer center nurse.
  • PTC collects the appropriate patient information.
  • Identifies a receiving provider or connects the referring provider with the preferred accepting provider.
  • Facilitates a physician-to-physician hand off; when an emergent patient is identified, transportation is immediately launched.
  • Coordinates bed placement and transportation.
  • Monitors the patient and communicates the patient clinical information to all appropriate parties.

Maintain and Sustain

With ongoing follow-up, our PTC RN’s will keep appropriate parties apprised of the patient’s status throughout the transfer process.

Information Needed to Initiate a Patient Transfer

Be prepared with the following information:

  • Patient's name, date of birth and weight.
  • Patient's primary diagnosis, required service and urgency.
  • Name and phone number of the sending physician.
  • Name of sending hospital, patient's location and phone number of nursing unit.
  • Requested accepting facility and physician, if available. If you do not have an accepting physician, the transfer center coordinator will help you identify one.
  • Patient's status:
    • Current condition/level of care.
    • Desired time for transfer.
    • Special bed and transport needs (isolation, ventilators, or IVs).
