Residency Verification Requests
Requests to verify residency training at the Family Medicine Residency Program at RWJUH Somerset in Somerville, New Jersey (formerly Somerset Medical Center) should be faxed to 908-704-0083. Please note that the only residency program based at RWJUH Somerset is in Family Medicine.
Requests must include:
- A signed Release of Information form
- Dates of enrollment in the residency program
- Instructions on how the information should be returned to the credentialing source
