You will be asked to update your insurance information when coming to the hospital. You are also required to have a valid form of ID.

Important Paperwork for the Hospital
Valid picture ID and any other necessary paperwork from the obstetrician’s office. Your OB will send a copy of the necessary paperwork to the hospital. Necessary paperwork includes: Hepatitis B, Group B Strep and HIV status documents.

Sample Birth Plan

Writing a birth plan gives you an opportunity to write out your wishes on how you would ideally like your baby’s birth to be handled. A written plan will also refresh your physician’s (or covering physician’s) memory when you are in labor. Download a sample birth plan provided by Saint Barnabas Medical Center.
