Woods Primary and Behavioral Health Center at Hamilton

Wood Primary and Behavioral Health Center at Hamilton

The Woods Primary and Behavioral Health Center at Hamilton, a collaboration between RWJBarnabas Health and Woods Services, delivers a first-of-its-kind care model in the region—offering both primary care and behavioral health services for individuals with autism, intellectual and developmental disabilities (IDD), and complex mental health conditions, as well as their families.

Services Offered

The multidisciplinary team includes a primary care physician, a primary care nurse practitioner, psychiatric nurse practitioners, and a consulting psychiatrist. Together, they deliver:

  • Primary care services including wellness exams, sick visits, and chronic disease management
  • Acute illness and minor injury care
  • Medication management
  • Immunization and screenings
  • Behavioral health care (coming in 2026)
  • Health navigation and coordination with specialists throughout RWJBarnabas Health

Patients with developmental or behavioral health needs often face fragmented care, traveling to multiple providers for different services. This center removes those barriers by providing coordinated, same-day access to medical and behavioral health care in a familiar, supportive environment.

  • Centralized, patient-centered care: All services under one roof, with providers collaborating daily
  • Specialized expertise: Staff trained in the unique needs of individuals with I/DD, autism, and mental health conditions
  • Family-focused approach: Support and education for caregivers

We welcome individuals age 6 and older who may benefit from specialized behavioral health and integrated primary care—whether they are managing chronic health conditions, addressing behavioral or emotional challenges, or seeking ongoing wellness support.

Located at Robert Wood Johnson University Hospital Hamilton, the Woods Primary and Behavioral Health Center is opening Fall 2025.
