Request a Lung Cancer Screening Appointment

Lung cancer is one of the most common forms of cancer in the United States and a leading cause of cancer death in both men and women, often because it is detected too late. A lung cancer screening with a low-dose computed tomography (CT) scan can detect cancer when it is most treatable.

Medicare, Medicaid and most insurance plans cover the cost of a lung cancer screening for individuals who meet the following criteria:

  • Adults aged 50 to 80 years who have a 20 pack-year smoking history and currently smoke or have quit within the past 15 years.

Individuals without insurance may still be eligible.

A prescription is required for a lung cancer screening. To request a lung cancer screening or assistance with obtaining a prescription, please complete the form below or call 888-637-9584.
