How to Become a One Source EAP Network Provider

To become a provider with the RWJBarnabas Health One Source Employee Assistance Program (SB EAP):

1. Click here for the Provider Requirements.
2. Contact the One Source Provider Relations Department at Network1@rwjbh.org or (800) 722-6976
3. Submit your credentials to begin the application process.
4. You will be sent an application to complete.
5. Once you have completed the Application process, you will be sent the Provider Orientation Packet.
