What should I expect during my visit?
Your visit will depend on the type of service scheduled (e.g., physical exam, drug screening, vaccinations, fit testing). Most visits include:
- Check-in and verification of information
- Completion of required forms (if not done in advance)
- Clinical services (exam, testing, etc.)
- Review of next steps or results (if applicable)
Please plan to arrive 10–15 minutes early to allow time for check-in.
What should I bring with me?
To ensure a smooth visit, please bring:
- Valid government-issued photo ID (e.g., driver’s license)
- Employer authorization form (if applicable)
- Commercial Driver’s License (CDL) – if you are scheduled for a DOT physical
- Any required paperwork provided in advance
- Insurance information (only if instructed)
- List of current medications
- Eyeglasses or contact lenses (if you wear them)
- Hearing aids (if applicable)
1. Why am I being sent to Employee Health / Occupational Health?
You may be scheduled for a visit due to pre-employment requirements, annual or baseline exams, job-related medical clearances, regulatory testing, return-to-work evaluations, or employer-requested services.
2. What should I bring to my appointment?
Please bring a valid photo ID, any forms provided by your employer, immunization records (if applicable), corrective lenses or hearing aids if you use them, and any required paperwork for your visit.
3. Do I need an appointment, or can I walk in?
Some services require appointments, while others may allow walk-ins. Follow the instructions provided by your employer or contact the scheduling team if you are unsure.
4. How long will my visit take?
Visit length depends on the services being performed. Most visits range from 30 minutes to 2 hours.
5. Will my employer see my medical information?
Your employer will only receive information necessary for job clearance (such as “cleared,” “not cleared,” or “cleared with restrictions”). Detailed medical information is kept confidential in accordance with HIPAA.
6. Will I be charged for my visit?
In most cases, your employer covers the cost of occupational medicine services. If payment is required, you will be notified in advance.
7. When will my results be available?
Some results are available the same day, while others may take several days depending on the test. Your employer will be notified according to their established process.
8. Who do I contact if I have questions after my visit?
If you have questions about your appointment or results, contact the Occupational Medicine/Corporate Care office listed on your paperwork.
9. What happens if I miss my appointment or arrive late?
Late arrivals or missed appointments may need to be rescheduled. Please contact the site or your employer as soon as possible if you are unable to attend.
