Stroke Discharge Transition Center

At The Stroke Discharge Transition Center, we are committed to providing you with education and resources to help increase your awareness of risk factors for stroke and hopefully reduce the chances of a second stroke.

Our nurse practitioners coordinate and facilitate a multidisciplinary approach that impacts the patient, ensuring the most efficient and effective goal-oriented services are provided including:

  • A post-discharge phone call within 24 to 48 hours of discharge.
  • Follow-up visit with a nurse practitioner (NP) for all stroke and transient ischemic attack (TIA) patients within 7 days after discharge to home from the hospital or rehab facility.
  • A one-hour encounter with the NP provides complete education including review of medications, test results, signs and symptoms of stroke, discussion of patient-specific risk factors, and ensures appropriate follow-up care.
