If your child is 8 to 17 years old, please have him or her complete the following form:

  • Patient Screen for Child Anxiety Related Disorders (SCARED)
    • It is recommended that children 8-11 years old completing the Screen for Child Anxiety Related Disorders (SCARED) questionnaire sit with an adult in case they have questions. Should you or child have additional questions regarding the questionnaire, your clinician will discuss questions and concerns with you at the time of your appointment.
