Formularios para el Nuevo Paciente

Bienvenidos al Hospital de Children’s Specialized. Por favor desplazarse hacia abajo al tipo de cita apropiado para acceder a los formularios de evaluación requeridos. Complete y envíe los formularios por lo menos 24 horas antes de su cita. Esta información ayudará a su proveedor a prepararse para su cita.

Para ayudarle a completer los Formularios para el Nuevo Paciente, necesitará lo siguiente:

  • Lista de proveedores médicos del niño
  • Lista de todos medicamentos y alergias
  • Historial clinica – incluyendo partida de nacimiento, cirugías, evaluaciones, terapias, etc.
  • Historial clinica de familiares
  • Educación/Escuela

Por favor, haz click en el tipo de cita que tiene y complete los formularios que se aplican a su niño/a lo mejor de su conocimiento. Tenga en cuenta que una vez que se envíe una sección, no podrá volver a cambiar su edición del contenido.
