Jersey City Medical Center is committed to connecting people to resources they need to improve their health. Sometimes, it's difficult to know where to start looking when a patient, client, friend or family member needs help.
The Patient Navigation Program wants to help point you in the right direction by providing information about resources that our area can offer, whether it's food, health, housing, job training, and much more.
Jersey City Medical Center has partnered with the New Jersey Health Care Quality Institute and Aunt Bertha, a public benefit provider, to develop a free, comprehensive, user-friendly social services search tool to assist you in connecting to needed services in Hudson County.
