In the forefront of patient care, three of our practices recently completed the rigorous application for designation as a Patient Centered Medical Home (PCMH), a designation for facilities that will transform the delivery of primary health care. The approval of the Center for Geriatric Health Care, Beth Prime Care, and Adult Health Center as Patient Centered Medical Homes make Newark Beth Israel Medical Center home to half of the PCMHs in the greater Newark Area.
