Patient and Family Advisory Council Application

Thank you for your interest in the Patient and Family Advisory Council (PFAC) at Clara Maass Medical Center. Patients and family members interested in being considered for an advisory position must complete this application, including interests and availability.

Selected applicants will be contacted to schedule an interview. Applicants selected for an advisory council position become volunteers of RWJBarnabas Health, and must abide by the health system’s patient privacy and confidentiality policies and procedures.

To ensure a diverse and independent perspective, individuals currently serving on Clara Maass Medical Center’s Board of Trustees are not eligible for PFAC membership.
