    The National POLST Paradigm is an approach to end-of-life planning based on conversations between patients, loved ones, and health care professionals designed to ensure that seriously ill or frail patients can choose the treatments they want or do not want and that their wishes are documented and honored.
  • Five Wishes
    There are many things in life that are out of our hands. A Five Wishes document, gives a person a way to control something very important – how they are treated if they become seriously ill. It is an easy-to-complete form that lets one say exactly what they want. It is the first living will that talks about the personal, emotional and spiritual needs along with a person's medical wishes.
