Graduation Requirements
- Successfully complete all competencies and activities during orientation

Obtain licensure from the New Jersey Board of Pharmacy as per
Responsibilities of the Resident (see page 7)
- Conduct a QI/research project and present the results at a regional residency
conference, e.g., ESRC, or at a predetermined state, regional or national meeting
- Present the QI/research project as an “in progress” poster
presentation at the ASHP Midyear Clinical Meeting or other suitable and
approved state, regional or national meeting
- Submit a manuscript suitable for publication by the end of the residency year
- Respond to at least one drug information question via electronic or written document
- Design and conduct at least one medication-use evaluation (MUE) or Failure
Modes Effects Analysis (FMEA)
- Prepare and present a formulary drug monograph or medication class review
for the P&T committee and/or other appropriate committee(s)
- Present at least one 1-hour ACPE-accredited presentation
- Provide at least one educational program to health care professionals.
The types of presentations will include patient cases, topic overviews
and research project ideas and results, or nursing/ physician in-services.
- Complete required didactic lecture(s) for the Ernest Mario School of Pharmacy
- Complete assigned Journal Clubs/topic discussions with pharmacy residents
and students
- Fulfill operational/staffing obligations of approximately every third weekend
for a total of 15 weekends (equivalent to 30 weekend shifts)
- Participate, e.g., take meeting minutes, present an agenda item, lead a
meeting, etc., in at least one of the health-system committees listed
on page 8 of this manual
- Support and participate in PGY2 MUSP recruitment efforts

Successfully complete all required learning experiences as per the
minimum threshold for summative evaluation scores

Minimum threshold for summative evaluation scores:
- A minimum of 80% ACHR of all required goals and objectives
- For any objectives not marked as ACHR, the resident must have been scored
as Satisfactory Progress for all required goals and objectives
