Course Description:

A two day symposium designed for a comprehensive review of extracorporeal membrane oxygenation (ECMO), temporary mechanical circulatory support (MCS), as well as the recognition and management of cardiogenic shock and respiratory failure. The first day (May 4th) will focus on the basics of ECMO, identification of respiratory failure, lung rescue, bridge to transplant, and the application of veno-venous (VV)-ECMO. The second day (May 5th), will focus on cardiogenic shock, temporary mechanical circulatory support (Impella), veno-arterial (VA)-ECMO. Sessions on special topics will include anticoagulation and peripheral support and critical care transport. Time will be devoted to “hands on” experience with the devices as well as a moderated group case discussion with an expert panel.

The symposium is geared toward physicians, residents, and fellows, in anesthesia, pulmonary, critical care, emergency medicine, internal medicine, and surgery, as well as perfusionists, respiratory therapists, pharmacists, and nurses who wish to broaden their expertise in this area.

Course Objectives:

  1. Discuss the basic principles underlying the physiology of extracorporeal life support.
  2. Discuss the different options for mechanical circulatory support for cardiogenic shock and respiratory failure.
  3. Identify cardiogenic shock and respiratory failure as well as the indications for mechanical support.
  4. Discuss key issues in management of patients on mechanical support.

  • General ECMO / tMCS concepts
  • The MCS circuit
  • Indication and selection for VV-ECMO
  • VV-ECMO as bridge to lung transplant
  • Cardiogenic shock
  • ECMO / Impella / ECPELLA: The tMCS decision tree for cardiogenic shock
  • ECMO / tMCS management / LV decompression
  • Bridge to transplant / LVAD / surgery / recovery / CMO
  • Rounding on the ECMO patient
  • Anticoagulation on ECMO
  • Technical aspects of cannulation for respiratory failure
  • Risks & complications related to MCS
  • Ambulation of the patient with temporary mechanical support
  • Too sick to transfer? Probably not. Critical care transport.
