Patient and Family Advisory Council Application

Thank you for your interest in the Patient and Family Advisory Council (PFAC) at Monmouth Medical Center and the Unterberg Children’s Hospital. Patients and family members interested in being considered for an advisory position must complete this application, including interests and availability.

Selected applicants will be contacted to schedule an interview. Applicants selected for an advisory council position become volunteers of RWJBarnabas Health, and must abide by the health system’s patient privacy and confidentiality policies and procedures.
