Leapfrog Safety Grade Fall 2025

West Orange, NJ, November 13, 2025 - The Leapfrog Group, an independent national nonprofit run by employers and other large purchasers of health benefits, awarded RWJBarnabas Health’s twelve hospitals high marks for patient safety in its Fall 2025 Hospital Safety Grades. The system continues to demonstrate strong performance and progress, with seven hospitals earning “A” grades and five earning “B” Hospital Safety Grades since Spring 2025.

Leapfrog assigns an “A,” “B,” “C,” “D” or “F” grade to general hospitals across the country using evidence-based measures of patient safety focused exclusively on errors, accidents, injuries, and infections.

RWJBarnabas Health’s A-rated hospitals are Community Medical Center in Toms River, Monmouth Medical Center in Long Branch, Monmouth Medical Center Southern Campus in Lakewood, Robert Wood Johnson University Hospital Hamilton, Robert Wood Johnson University Hospital Rahway, Robert Wood Johnson University Hospital Somerset, and Trinitas Regional Medical Center in Elizabeth. Clara Maass Medical Center in Belleville, Cooperman Barnabas Medical Center in Livingston, Jersey City Medical Center, Newark Beth Israel Medical Center, and Robert Wood Johnson University Hospital in New Brunswick each received a “B” grade.

“Our commitment to quality and patient safety cannot be overstated, and these scores exemplify how RWJBarnabas Health continues to drive performance improvement across all our hospitals,” said Andy Anderson, MD, Executive Vice President and Chief Medical and Quality Officer at RWJBarnabas Health. “Ensuring that every patient receives safe, compassionate, and high-quality care every day is our top priority.”

Robert Wood Johnson University Hospital and Trinitas Regional Medical Center both improved a letter grade.

“Earning an ‘A’ Grade means RWJBarnabas Health made a true commitment to put patient safety first,” said Leah Binder, president and CEO of The Leapfrog Group. “We congratulate the leadership, Board, clinicians, staff and volunteers that all had a role to play in this achievement.”

This was Robert Wood Johnson University Hospital Rahway’s ninth straight “A” rating and the sixth in a row for both Community Medical Center and Monmouth Medical Center Southern Campus.

The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries that kill more than 500 patients a day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are updated twice annually, in the fall and spring.
