You will find information below regarding our days and hours of operation, telephone number and e-mail address for questions as well as our address for payment remittance.

  • Patient name

  • Date of birth

  • Patient guarantor

  • Telephone number

  • Times you can be reached

We will make every attempt to respond to your e-mail within one business day. If we are unable to gather the information necessary to respond to your question within one day, we will respond with an estimated time frame of when an answer will be available.

All requests for information will be verified through our billing system and mailed via US Postal mail. Due to Federal regulations, we will not respond to requests via e-mail.

*Please note: Due to the nature of our business, patient confidentiality is our number one concern, therefore, certain questions regarding diagnosis of the patient, tests performed on the patient, etc. cannot be answered.

To make inquiries or payment in person, our cashier office is located in the main lobby of the hospital and is open Monday to Friday 8:30 a.m. to 5 p.m., for your convenience.
