Diabetes Self Management Series - Community Medical Center
A 4 session education program focusing on what diabetes is and how it affects your body and what you need to know about what you can eat to maintain good blood sugars.
To attend you will need:
- A prescription from your doctor for Diabetes Education with Diagnosis Code
- Your Health Insurance information
You may bring a guest with you at no additional charge.
