RWJBarnabas Health, in partnership with Montgomery Medical Associates, is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a collaborative practice serving Somerset County, NJ. They will deliver high-quality primary care services in a small, community-based family practice with additional coverage at a nearby nursing facility. This role functions with a high level of autonomy while collaborating closely with physicians and the interdisciplinary care team to assess, diagnose, treat, and manage patient care across settings.
This is a part-time position (20 hours per week) ideal for an experienced APP seeking work-life balance without sacrificing meaningful patient impact.
Schedule Requirements
Part Time – 20 Hours/Week (2½ Days)
- Tuesday: 9:00 AM – 5:00 PM
- ½ day in office
- ½ day in nursing facility
- Wednesday: 9:00 AM – 5:00 PM
- ½ day in office
- ½ day in nursing facility
- Thursday: 9:00 AM – 12:00 PM
- Office only
- Optional: Potential for one Saturday per month (9:00 AM – 12:00 PM)
Pay Transparency
The hourly rate reflects the anticipated compensation for this position if hired to work in New Jersey.
Hourly Rate: $65.00 – $80.00 per hour
Compensation will be determined based on factors including clinical experience, licensure, and professional background.
Certifications and Licenses Required
- Active New Jersey Nurse Practitioner or Physician Assistant License
- CDS and DEA (required)
- Minimum 1 year of APP clinical experience
Qualifications
Required:
- Minimum of one (1) year of experience as an APP
- Active New Jersey NP or PA license
- CDS and DEA registration
- Strong clinical assessment and decision-making skills
- Excellent verbal and written communication skills
- Ability to work independently while collaborating with physicians and staff
Preferred:
- Experience in primary care, family medicine, or post-acute/nursing facility care
- PointClickCare experience
Essential Functions
- Perform comprehensive patient evaluations including medical histories and physical exams
- Diagnose and manage acute and chronic conditions
- Develop and implement individualized plans of care
- Provide continuity of care between office visits and nursing facility coverage
- Order, review, and interpret laboratory and diagnostic studies
- Prescribe medications in accordance with federal and state regulations
- Document patient encounters accurately and timely in the EMR
- Collaborate with physicians, nursing staff, and interdisciplinary team members
- Educate patients and families on disease management, medications, and preventive care
- Participate in quality improvement initiatives and protocol development
- Other duties as assigned
