Support on the Journey: Perform Care Eligibility and Services
Support on the Journey is a free online support group for caregivers of children diagnosed with Autism Spectrum Disorder.
Perform Care Eligibility and Services will be presented by:
- Department of Human Services of Monmouth & Ocean Counties
- Monmouth County Family Support Organization
- Case Management Organizations of Monmouth & Ocean Counties
This session will focus on the following:
- What is Perform Care (also known as NJ Children's System of Care)
- Services and eligibility process to obtain support
- How to respond to questions within the application
- How to successfully complete and submit the application
- Services offered based on age (18 and younger; 18 to 21)
- How to transition to adult services before age 22
