Community Health Needs Assessment

Trinitas Regional Medical Center collaborated in a comprehensive Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in the Medical Center's service area, made up primarily of Elizabeth, New Jersey and the surrounding towns. The purpose of the CHNA is to gather information about local health needs and health behaviors. The assessment examines a variety of indicators including risky health behaviors and chronic health conditions.

The completion of the CHNA enables Trinitas to take an in-depth look at the health needs of the community it serves. The findings from the assessment are being utilized by Trinitas to prioritize community health issues and develop an implementation plan to direct community health improvement efforts.
