Patient Navigation

The J. Phillip Citta Regional Cancer Center offers a Patient Navigator Program free of charge to all cancer patients receiving oncology services at Community Medical Center.

Since cancer is a complex disease that impacts a person’s life in so many ways, the program is designed to guide patients and caregivers as they face the psychological, emotional and financial challenges that cancer brings.

Patients are provided with individualized information and services to help them navigate their way through the health care system along their cancer journey — from diagnosis and treatment through to recovery and survivorship.

Community Medical Center “navigators” help people facing cancer by providing information on cancer and treatment options, community resources, and referrals to appropriate agencies and persons, among a host of other areas of assistance.

Patients who enroll in the confidential service will receive a personal phone call from a counselor and a mailed packet of information that details the various aspects of the program from which they can choose.
